There a few different ways that we can begin filling your prescriptions.

One way is to simply bring in your next prescription that you receive from the doctor. There is nothing more you need to do than that--we'll take care of all the rest.
Another way is to tell your doctor to send your prescription electronically to our pharmacy. We'll have it in the works before you even arrive and simply ask you a few questions to finish up.
A third way, if your current prescription has refills remaining (and you won't be seeing the doctor soon) is to use our Transfer Form below.
